THE FORT FRANCES FAMILY HEALTH TEAM IS NOT ACCEPTING NEW PATIENTS
A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient.
Designed to improve and expand access to primary care, our Family Health Team consists of nurse practitioners, nurses, a dietitian and youth mental health worker that work collaboratively with our physician group, so that you receive the very best care, when you need it, as close to home as possible.
The Fort Frances Family Health Team (FF FHT) provides service to patients in Fort Frances and surrounding area. Our programs and services are available to individuals with or without a family physician and include primary and preventative health care.
FF FHT programs and services support health promotion and improved chronic disease management through treatment, monitoring and skill development for patient self-management.
When visiting the clinic we ask that you wear a mask or face covering if you have cold / flu like symptoms.
The Walk-in Clinic has been cancelled until further notice.
